Please complete and submit the following forms to Mark for the 2009-2010 soccer year:
- EPYSA RG6 player participation form .
- Spirit player registration form
- Spirit Medical release form (need a copy of the front and back of your medical card)
- Spirit Parent conduct form
- Spirit Photo release form.
Also needed is a 1" x 1" color photo for the ID card and a copy of your sons' birth certificate.
Please submit all 5 forms, the picture and the birth certificate copy at the same time.
There is a $250 fee to Spirit for registration, and the usual $150 tournament fees. Please make checks payable to SUSC 92/93 Boys Real. |
